By submitting this video to MEDISTIK Inc. (MEDISTIK), I confirm that I own the rights for all content contained within this video with the authorization to publish, distribute and assign such content. I also agree that I will make no monetary or other claim against MEDISTIK for the use of the video and that this video will become the property of MEDISTIK upon submission.
At any point if I want MEDISTIK to stop using my video, I can email MEDISTIK at info@medistik.com or call the company at 1-855-347-0226 and my video will be removed from any existing or further use, as is possible at the time, and the file will be deleted by MEDISTIK.
With this submission, I am irrevocably authorizing MEDISTIK, its related parties and authorized representatives to copy, exhibit, publish or distribute the video as is, or in a modified version, in connection with publicizing and promoting MEDISTIK and/or Soldier On or for any other lawful purpose. The video and the statements contained within may be used on websites, publications or in any other distribution media.
I hereby hold harmless and release MEDISTIK from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
